In the WordPress dashboard go to Ninja Forms submenu and click on Add New.
Select your page template here.
Here drag & drop fields that you want in your form.
Then click on the field to edit properties.
click on publish button.
To use these form Copy the shortcode & paste it on the pages where you want to show your form.
