In WordPress dashboard go to WPForms submenu and click on Add New.
Select your page template here.
Click on the Setting button than General.
Fill the required fields such as form name, description, form CSS class etc.
Click on Fields than Add field.
Drag & drop fields that you want in your form.
After dropped fields, you may also change their labels or properties.
Click on SAVE button.
